Form I-5.<div><br></div><div>In overall good condition with some age-related spotting on front page. A hole was punched in the lower left corner. Booklet has 4 pages with each open page measuring 11" by 8.5". When folded in half, the document measures 8.5 inches by 5.5 inches. Weight is 0.4 ounce.</div></div><div style="font-size: 18.6667px;"><br></div><div style="font-size: 18.6667px;">Stored in a smoke-free, climate-controlled home.
